“PERSONAL CARE PLATINUM

PERSONAL HANDS-ON CARE (A minimum of (1) Geriatric Care Manager visit weekly is needed with any request)
 
BENEFITS & CHOICES:

  • Assistance with bathing, showering, dressing, hygiene and grooming
  • Assistance eating, mobility & transfer, and with toileting or diapers
  • Assistance with pill bottles and pill boxes, etc.
Senior Care - Quality of Life Service

“MAXIMUM EMBRACE” QUALITY OF LIFE SERVICE

(A minimum of (1) Geriatric Care Manager visit weekly is needed with any request)
 
BENEFITS & CHOICES:

  • Assistance with hobbies, projects, crafts, and hosting home parties or a get-together
  • Assistance with local sightseeing, theater, concerts, plays, etc.; Assistance with attending formal engagements and events
  • Assistance with dining out; coffee bar, dessert cafe, country club, etc.
  • Assistance reaching/attaining ‘Bucket List’ goals
  • Assistance/encouragement to attend lodge meetings, club meetings, trade meetings, civic meetings, library and book club events
Senior Care - Alzheimer's and Dementia

ALZHEIMER’S CARE / DEMENTIA CARE

(A minimum of (2) Geriatric Care Manager visits weekly are needed with any request)
 
BENEFITS & CHOICES:

  • Receive only care aides who have had special training in Alzheimer’s & Dementia Memory Care
  • Receive research-based memory care that focuses on patient dignity, safety, calmness, tranquility, and social interaction
  • Receive care centered on mind-stimulating activities as well as honoring who the senior was earlier in life
  • Special support and training for the family of the patient including: hands on training, referral to community resources like AA
  • Photo album sessions, guided one on one music time; Memory Lane discussion activity, preparation for dementia-safe home
Elder Care - Morning Assistance

“RISE N SHINE SERVICE”

(A minimum of (1) Geriatric Care Manager visit weekly is needed with any request)
 
BENEFITS & CHOICES:

  • Receive gentle wake up at bedside or greet aide when they come in (can have breakfast tray in bed as a wake up greeting)
  • Receive assistance with morning care; oral care, facial washing or shower, nail care, hair grooming, etc.
  • Receive assistance with dressing and clothes selection; handed clothing for self-dress, or can be assisted with partial or full dressing
  • Have breakfast in any location of choice
Senior Care - Evening Assistance

“EVENING TURNDOWN SERVICE”

(A minimum of (1) Geriatric Care Manager visit weekly is needed with any request)
 
BENEFITS & CHOICES:

  • Have herbal bedtime tea with relaxing music, Receive assist with sleeping attire
  • Receive assist with oral care, Receive a back rub/light massage with/without oils or lotion
  • Have bed covers pulled down, Be assisted to a warm bath soak
  • Have a soothing story/article/chapter read aloud
Senior Care - Nutritious Meals

“WAITER & NAPKIN SERVICE”

(A minimum of (1) Geriatric Care Manager visit weekly is needed with any request)
 
BENEFITS & CHOICES:

  • Receive registered nurse nutritional teaching/counseling and/or dietary consult with dietitian
  • Menu selection made in conjunction with client’s preferences well in advance for grocery store trips & purchases
  • Nutritious, well-balanced meals are planned, prepared,  and positioned attractively on a tray
  • Meal is brought out to setting of choice; al fresco, in front of the television, in the den/study, or in bed
  • Complete cleaning of kitchen area used, as well as utensils, plates/bowls, stemware, silverware after each meal
