 BENEFITS & CHOICES:

  • We'll provide initial and ongoing medication teaching; create a comprehensive medication list and ongoing maintenance of medication list updates
  • Receive medication compliance monitoring, have any non-compliances addressed with the patient and responsible party; explore solutions to remedy any problems found
  • Your personal Geriatric Care Manager will inquire about polypharmacy issues with the primary care physician and will make recommendations
  • Your personal Geriatric Care Manager will notify the primary care physician of any new medications added from a specialty, Urgent Visit, or Emergency Room physician
Senior Care - Medication Assistance

“MED MANAGEMENT PLATINUM

 BENEFITS & CHOICES:

  • Receive IM (directly into muscle) or SQ (under the skin) injections per physician orders (daily, weekly, or monthly)
  • The pharmacy will be called to have all prescriptions refilled, picked up, and then delivered to the patient’s home
Senior Care - Relocation Assistance

“MOVING FORWARD” (LATE LIFE RELOCATION ASSISTANCE)

 BENEFITS & CHOICES:

  • Assistance with moving from a nursing home or Assisted Living Facility back to home, or from home to a facility type residence
  • Have furniture and personal belongings moved via trucking or van service to a new location
  • Help with airline or ground transportation along with appropriate level of professional escort to provide assistance with
    oxygen, medications, walking cane or walker, incontinence, dementia, glucometer and nutritional care
  • Receive assistance with obtaining a new contact or resource person to assist at a new location if it's a non-facility transfer
Senior Care - Outpatient Surgery

“DAY SURGERY HOME CARE PLUS”

 BENEFITS & CHOICES:

  • An RN will pick up the patient and transport them to a same day surgery center or hospital, and then wait during entire surgery/procedure
  • The RN will interface with health care surgical team for questions and instructions, and then pick up new prescriptions from the pharmacy
  • An RN will take the patient home; help the patient into bed and assist with post-surgical aftercare; dispense new medications and check vital signs
  • The RN will check and reinforce surgical dressings; assess for infection, excess bleeding complications, provide appropriate nursing care, and follow up with doctor via phone for further instruction
Senior Care - Wellness Checking Service

THE TOUCHBASE PROGRAM:

“TELECARE” SERVICE
(TeleCare =8 minute minimum per phone call)
 
BENEFITS & CHOICES:

  • "TeleCare:" Receive a phone call from a nurse several days per week to check on the status of the patient to see if there is need for a visit to check things out in person

 

“HOMECHEK” SERVICE
(HomeChek =15 minute minimum visit per day)
 
BENEFITS & CHOICES:

  • Receive a home visit from a nurse or Geriatric Care Manager several days per week to check on the patients status, well-being, hygiene, and personal appearance; Provide a Mini-Home safety inspection and monitoring; Do a refrigerator and pantry abundancy check and inspect the food expiration dates, etc.
  • Mail check: See if bills are being paid; be sure there are no mail pile ups present; Laundry check: check for clothes pile ups and be sure the patient is wearing clean clothes
  • Medication compliance check: pill box/carousel, check prescription bottles

“MD APPOINTMENT CONSOCIATE”

(Getting the absolute most out of a typical doctor visit)

A Geriatric Care Manager provides the following:

  • Physician appointment escort and acts as patient transport, patient advocate, and communication liaison
  • Requests polypharmacy changes to eliminate duplicate medications and increase quality of care
  • Asks physician to consider alternate medication options, treatment options, referrals to specialty doctors, etc.
  • Brings home details, signs and symptoms, unreported concerns, and physical changes to the doctor’s attention
  • Maximizes physician time in the exam room/office so ALL concerns/issues/problems are addressed at one time
